Prior to your Appointment:

We utilize a paperless intake process. Prior to your appointment you will be emailed all the necessary information and intake documents. Please have these documents complete prior to your first appointment.

Cancellations:

We get it. Things come up. If you are not able to attend your scheduled appointment please let us know at least 24 hours in advance to avoid any cancellation charge.

Payment:

All copays and payments are due at the time of your appointment. We accept cash, credit, or venmo.
